Great Sutton Medical Centre is a large GP practice with 20,000 patients based in the suburbs of Ellesmere Port. We are the largest practice in the area, which means we are always busy so every day is different.
Main duties of the job
Salaried GP 4 sessions delivering a high standard of primary care to our patients (via various methods including, telephone, face to face consultations and e-consultation) Effective management of patient's long term conditions, Processing of administration including referrals, prescription requests and other associated tasks, Work collaboratively, accepting an equal share of the practice workload Contribute to the successful implementation of continuous improvements, development and quality initiatives within the practice Attend and contribute to weekly clinical meetings and other practice meetings Committed to achieving practice targets opportunistically during regular consultations

Responsibilities and Duties -
KEY RESPONSIBILITIES:
Manage a clinical caseload. Take part in leading on key disease areas (monitoring trends in the Quality and Outcomes Framework treatment, staff training and prescribing). Assist in developing appropriate healthcare services for a diverse, patient list (practice based commissioning, patient participation and health promotion). In accordance with the practice timetable, as agreed, the post-holder will be available to undertake a number of clinical sessions including queries, visiting patients at home, checking and signing repeat prescriptions, actioning test results, medical reports and examinations (e.g. Insurance Companies) on behalf of the Practice. These duties should be performed in a timely manner using agreed protocols. Act upon most alerts (e.g. QOF) and reminders during a routine consultation i.e. Provide all GP services required as part of the GMS contract held between Great Sutton Medical Centre Partnership and the commissioning CCG, including all core services such as Child Health Surveillance. Undertake surgeries in accordance with the access needs of the practice population and the contractual obligation of the practice. Respond to medical problems presented by patients, including history taking, examination, investigation, diagnosis, treatment and referral where appropriate.
Attend clinical meetings on a regular basis, to chair these and present clinical topics as required for the maintenance of educational standards and high quality care. You will receive feedback from clinical partners via peer review and to act upon recommendations and demonstrate change. Meet all tight timescales/deadlines for audits and written returns to ensure that the Practice meets quality standards and receives the designated funding (e.g. Quarterly Enhanced Services returns, annual QOF audit etc.).
